Accessing the service

Find out how to access Pilgrims Hospices’ services here.

Referrals to any of Pilgrims Hospices’ services can be made by a GP, Consultant, Clinical Nurse Specialist, District Nurse or Community Matron. You should ask them in the first instance if you think you should be referred.

After your local Hospice has received the referral, a member of the hospice team will be in touch to arrange an appointment with you – either at your home, or a clinic appointment at the hospice (similar to a GP appointment). The hospice team does not take over your care, but instead works with your GP and other health care professionals to care for and support you and your family.
