Community care

If staying at home with family and friends is what you would prefer, Pilgrims Hospices community team will help you try and achieve it.

Your care at home is still managed by your GP but could be supported by a Pilgrims Hospices Community Nurse, an out-patient appointment at your local hospice, attendance at a tailored Living for Today programme, or telephone support.
