Improving End of Life Care
We joined with the Strategic Health Authority to fund research to improve End of Life Care across east Kent.
Project Invicta is a two year project which commenced in April 2012 in response to the needs of patients and carers in east Kent. The project was planned after listening to patients and staff about how to improve care. Although there were many examples of good practice with some patients and carers having very good experiences, it was also reported that there were concerns about gaps in the service provided to patients and their carers over the whole 24-hour period.
So far, Project Invicta is already improving end of life care by providing a Care Navigation Centre which is available 24 hours a day, 7 days a week to co-ordinate and navigate care for a patient at the end of their life. Patients and carers can ring one number where services will be coordinated to ensure the patient or carer gets the support and care they need.
The Care Navigation Centre will work hard to put patients at the centre of their care and provide better-coordinated services by improving communication between GP, hospital, hospice social services and community teams. The service is supported by “My Wishes” an electronic network that allows all health care professionals involved in a patient’s care to share the record.
