If your patient is in the later stages of an incurable illness and has needs that can't be managed in a primary care, care home or hospital setting, they may benefit from a referral to Pilgrims Hospices.
To discuss whether your patient is eligible for a referral, you can call our Professional Advice Line on 01233 504 133 or contact our team direct.
We offer a wide range of services and will work in partnership with you to support your patients and their families.
Pilgrims services include:
Pilgrims Therapy Centres
We run a range of Wellbeing and Social Programmes from Pilgrims Therapy Centres in Canterbury, Ashford and Thanet. These programmes have been designed by our experts to help patients with incurable illness to manage their physical and psychosocial symptoms such as pain, breathlessness, fatigue, anxiety, muscular stiffness, and reduce the risk of falls. We also offer programmes for family and carers. We offer these interventions for a specific number of sessions, at the end of which we will discharge the patient unless our assessment highlights further needs.
Specialist palliative care services
A referral to our community services, outreach clinics, or one of our three inpatient units in Canterbury, Ashford, or Thanet provides patients with access to the full range of specialist services available through Pilgrims Hospices. Our multidisciplinary teams work together to deliver expert symptom management, holistic assessment, and personalised support for patients living with incurable or life-limiting illnesses.
Our staff have extensive expertise in palliative and end-of-life care, supporting patients with complex physical, emotional, social, and spiritual needs. Care is provided by a skilled multidisciplinary team that includes doctors, nurses, occupational therapists, physiotherapists, counsellors, complementary therapists, social workers, and spiritual care professionals, ensuring patients and their families receive comprehensive, person-centred support throughout their journey.
Hospice at Home
Our Hospice at Home service provides specialist palliative and end-of-life care for patients in the comfort of their own homes across east Kent. Working in partnership with external health care professionals, the team delivers holistic, hands-on care, legacy work, and practical and emotional support for patients in the last 72 hours of life, as well as their families, and carers. Referrals are reviewed promptly, with the aim of responding on the same day where appropriate, ensuring timely access to compassionate hospice care.
The service is delivered by a dedicated team of Senior Healthcare Assistants, supported by the wider multidisciplinary team at Pilgrims Hospices, including specialist nurses, doctors, therapists, counsellors, and social workers. Operating between 07:30am and 9:00pm, seven days a week, Hospice at Home supports patients living within the Canterbury, Ashford, and Thanet areas, helping them to remain comfortable, maintain their dignity, and receive care in their own home.
