If your patient is in the later stages of an incurable illness and has needs that can't be managed in a primary care, care home or hospital setting they may benefit from a referral to Pilgrims Hospices.

To discuss whether your patient is eligible for a referral you can ring our Professional Advice Line 01233 504133 or contact our team direct.

We offer a wide range of services and will work in partnership with you to support your patients and their families

Pilgrims services include:

Pilgrims Therapy Centres

We run a range of Wellbeing and Social Programmes from Pilgrims Therapy Centres in Canterbury, Ashford and Thanet. These programmes have been designed by our experts to help patients with incurable illness to manage their physical and psychosocial symptoms such as pain, breathlessness, fatigue, anxiety, muscular stiffness, and reduce the risk of falls. We also offer programmes for family and carers. We offer these interventions for a specific number of sessions, at the end of which we will discharge the patient unless our assessment highlights further needs.

Specialist palliative care services

A referral to our community service, outreach clinics or to one of our three in patient units in Canterbury, Ashford or Thanet gives patients access to the whole range of Pilgrims professionals for support and symptom management.

All our staff have expertise in caring for patients with a terminal or life limiting illness who may have palliative needs, difficult symptoms or circumstances. We have nurses and doctors available plus occupational therapists, physiotherapists, counsellors, social workers and a spiritual care team.

We can also help you work with patients and their family to talk about Advance Care Planning for end of life care. Read More.
