The team talks to each person about how their loved one's illness is affecting them and their family, then looks at what support Pilgrims can provide.
Pilgrims whole multidisciplinary team of nurses, physiotherapists, occupational therapists, social workers, chaplains, consultants and doctors offer an individual package for you and your loved one that gives them care and comfort along their whole journey. You can access this support in your own home, at a Pilgrims Outreach Centre, in a care home or hospital or on one of our wards.
Our services are all holistic, that means we look at each person’s physical and practical needs, and at their psychological and emotional symptoms. We continue this support after bereavement too.
When your family member or friend is referred to Pilgrims services, the Pilgrims team will also talk with you to understand your individual needs.
