Best Practice Forum


We are excited to announce that we have recommenced the Best Practice Forums led by Pilgrims Hospices’ Multi-Disciplinary Team.

These forums give us the opportunity to share expert knowledge and experience in supporting people with life-limiting conditions.

Finding ways to collaborate, bring deeper awareness and demonstrate the importance and encouragement of early referrals, will aid in improving communication/collaboration with external healthcare professionals in east Kent and beyond.

Best Practice Forums will offer the opportunity for all health and social care professionals and students to gain a greater understanding and awareness of how working in collaboration with the providers of specialist palliative care will not only improve their roles, but most importantly improve and promote the effective and appropriate end-of-life care for all patients.

We will be discussing the following topics:

  • The importance of identifying the 1%
  • Advance planning and communication, including the ReSPECT plan and process
  • End of life is for living; not just dying

There will also be a Q&A session followed by an open discussion focussing on the challenges in engaging patients to do their advance care planning.

Together, we can improve end-of-life care for everyone.
