Course description

We’re pleased to announce the launch of another national programme for care homes for the GSFCH training programme.

This Quality Improvement training programme aims to improve the supportive care for all residents as they near the end of their lives.

This is the largest and most comprehensive end of life care training programme for care homes in the UK, focusing on improving quality of care, collaboration with GPs and others to reduce hospitalisation. It is widely-acclaimed and endorsed by all leading care homes organisations, the Department of Health and thousands of care home staff, as a key means of improving care for residents nearing the end of life.

Using a structured approach, centrally coordinated by the GSFCH team and personally facilitated by a local facilitator, this programme will make a real difference to the care home, and will enable residents to live well and die well in the place and manner of their choosing.

The aims of the GSF care home programme are to:

  • Improve the quality of care for people approaching the end of their life.
  • Improve collaboration with GPs / Primary Care Teams and Specialist Care Teams.
  • Reduce hospitalisation in the last stages of life, enabling more residents to die in the Care Home.

The programme requires one or two nominated leads for the GSFCH in the home to:

  • Attend all the interactive workshops;
  • Share examples of good practice with others;
  • Take part in evaluation, e.g. After Death Analysis;
  • Lead and be responsible for the implementation of the programme in the home, in order to cascade the learning from the workshops to others in the home.

Criteria for inclusion on  the programme:

  • Senior support and commitment from managers, owners, CCG etc., to continue to sustain improvements from the programme;
  • Commitment from same staff member/s to attend all workshops.
